Equipment Purchase Approval Request

This request form is used to seek approval prior to incurring costs when the total amount equals or exceeds the Department specific threshold (see lesson for thresholds).

NOTE: CDE allows purchases from a pre-approved equipment list without written pre-approval if the total cost is $20,000 or less (see lesson for pre-approved list).
