Monitoring Review Evidence
Documentation of the contractor’s efforts to facilitate relationships between parents and LEAs or regional centers. Documentation may include referrals, meeting agendas, etc.
Documentation of solicitation efforts within the community, such as for donation of goods and services.
Documentation such as flyers, social media links, web pages, etc., that identify how information of the services offered by the program are being shared with the community.

Community Involvement
Facilitating Relationships
Connections must be made with local educational agencies (LEAs), SELPA’s, or regional centers in order to facilitate relationships between parents of children with disabilities.
What this process might look like in a program:
Program staff support families by issuing referrals to LEAs, SELPA’s or regional centers and maintain records of referrals, meeting agendas, etc.
Soliciting Community Support
Contractors should solicit support from the community, such as for donation of goods and services and maintain documentation of solicitation efforts from the community.
What this might look like in a program:
Program staff develop & maintain letters requesting donation of goods and services.
Program staff maintain documentation of efforts as indicated on an in-kind donation form that was provided to the individual or company who donated an item or funds to your organization.
Information Sharing
Contractors must identify how information of the services offered by the program are being shared with the community.
