The program annually distributes a parent survey to parents and analyzes the results to plan and conduct activities to help parents support their child's learning and development and to meet the family's needs.
The survey is utilized as part of the annual self-evaluation process.

Parent Survey
The parent survey is a questionnaire completed by the parent to assess the child care program or services that the child and family receive.
Step 1: Distribute & Collect Survey
The Desired Results Parent Survey is distributed annually as part of the Program Self-Evaluation process. Parents may choose to complete the survey on paper or electronically.
The parent survey asks for information about how the program helps parents support their child’s learning and development and meets the family’s needs.
Step 2: Tally Results
Refer the family to appropriate agencies in the community based on the health or social service needs.
What this might look like in a program:
Parent completes their initial application & identifies a need for dental referrals. The Enrollment Specialist identifies referrals & mails the copy of the Health & Social Services form to the parent along with their Notice of Action to Approve Services.
NOTE: It is important to form good relationships with other community agencies, as you will be able to assist the parent quickly & effectively
211 is a great resource. This resource is administered by United Ways across the nation.
ConnectIE is another great health & community resource that is free and user-friendly. This tool helps agencies provide referrals to improve outcomes for those in need.
For the San Francisco & Los Angeles area, One Degree is a great way to access resources that may not be readily available or known to the general public. It is also a great way to stay informed & connected to available resources in the area.
Step 3: Follow-Up
Follow-up with the parent to ensure that their needs have been met.
What this might look like in a program:
30 days following the issuance of the referrals, the Enrollment Specialist follows up with the parent to ensure that their needs have been met.
Follow-up is documented on bottom of Health & Social Services form
NOTE: Remember to have a system to document any follow-up as this will be requested in a review.
The regulations do not indicated the timeline for following-up with a family. Typically agency staff follow-up within 30 days of a referral being given to the parent.
Supplemental Training
Ages & Stages Questionnaires (ASQ)
ASQ’s are commonly used by agencies in partnership with parents to identify developmental delays. This supplemental training is designed to support staff that work with families to complete the screening.
Ages & Stages Questionnaires
Early identification of infants & children who have developmental delays or disorders is key.
Note that there are various developmental screening tools used to meet program requirements. The Ages & Stages, 3rd edition (ASQ-3) AND ASQ Social Emotional, 2nd edition (ASQ: SE-2) are commonly used throughout the early education field.
