Requirement
The program annually distributes a parent survey to parents and analyzes the results to plan and conduct activities to help parents support their child's learning and development and to meet the family's needs.
The survey is utilized as part of the annual self-evaluation process.

Parent Survey
The parent survey is a questionnaire completed by the parent to assess the child care program or services that the child and family receive.
Step 1: Distribute & Collect Survey
The Parent Survey is distributed annually as part of the Program Self-Evaluation process.
Parents may choose to complete the survey on paper or electronically. Many contractors use a hybrid model that gives a parent the option of completing a survey by either paper or electronically.
The departments have a sample survey located at desiredresults.us. If a contractor develops their own survey form, it must include the basic content listed on the departments Parent Survey form. Monarch Link’s sample survey is located within the Parent Survey Kit.
Step 2: Tally Results
After surveys are returned, results are tallied by program.
TIP: Using an electronic survey, such as through Google Forms or Survey Monkey, can save a program a lot of time as the results are electronically tallied vs. tallied by hand.
Step 3: Analyze
Based on the tallied results, key findings are identified and documented on the Planning Worksheet: Summary of Findings.
A Parent Survey Summary of Findings is located within the department's survey instructions, or you can make your own as long as it contains the minimum data elements.
Step 4: Action
Attainable action steps are developed and carried out, as written on the Summary of Findings planning worksheet.
Action steps are intended to support the program in progressing from Not Satisfied to Satisfied OR Very Satisfied AND No to Yes.
Be sure to include modifications in communication, training, support, resources, health and safety, and child development.
Step 5: Follow-up
At the time of planning, follow-up dates will be added to the program calendar.
In addition, during the following program year self-evaluation, data will be analyzed to see if the identified area had improved. If no improvement, adjustments will be made.
In the end, don’t forget to document any follow-up and reflections on the Parent Survey Summary of Findings form.
